Description
Bacterium that causes tularemia (rabbit fever)

Structure
Francisella tularensis is a small, Gram-negative, facultative intracellular coccobacillus belonging to the family Francisellaceae. The organism is highly infectious, with exposure to very small numbers of bacteria capable of causing disease. Francisella tularensis is maintained in nature through complex transmission cycles involving rabbits, hares, rodents, ticks, deer flies, and other wildlife species. Several subspecies have been identified, with Francisella tularensis subsp. tularensis (Type A) and Francisella tularensis subsp. holarctica (Type B) being the primary causes of human disease. Following entry into the host, the bacterium survives and replicates within macrophages and other phagocytic cells, enabling dissemination to lymph nodes and multiple organ systems. Due to its high infectivity, environmental stability, and potential for aerosol transmission, F. tularensis has been classified as a Category A biothreat agent.

Diagnosis
The diagnosis of Francisella tularensis infection is performed through serological, molecular, microbiological, and histopathological methods. Polymerase chain reaction (PCR) assays provide rapid and sensitive detection of bacterial DNA from clinical specimens including lymph node aspirates, ulcer exudates, respiratory samples, and tissue biopsies. Serological testing, including microagglutination and ELISA-based assays, is widely used and often demonstrates a significant rise in antibody titers during the course of infection. Culture of F. tularensis is possible but requires specialized laboratory facilities and biosafety precautions due to the organism's high infectivity. Histopathological examination may reveal granulomatous inflammation and necrotizing lesions. Early diagnosis is important because prompt antimicrobial therapy significantly improves outcomes.

Symptoms
Tularemia typically develops following an incubation period of approximately three to five days but may range from one to fourteen days depending on the route of exposure. Clinical manifestations vary according to the site of infection. Ulceroglandular tularemia, the most common form, is characterized by a skin ulcer at the site of inoculation accompanied by painful regional lymphadenopathy, fever, chills, fatigue, and headache. Glandular tularemia presents with lymph node enlargement without an obvious skin lesion. Oculoglandular disease results from ocular exposure and causes conjunctivitis with regional lymphadenopathy. Oropharyngeal tularemia may develop after ingestion of contaminated food or water and is associated with sore throat and cervical lymphadenopathy. Pneumonic tularemia, acquired through inhalation or secondary dissemination, can cause cough, chest pain, shortness of breath, and severe pneumonia. Without treatment, systemic infection may progress to sepsis, multi-organ involvement, and death.
