Description
Virus that causes dengue fever and severe dengue

Structure
Dengue virus (DENV) is an enveloped, positive-sense, single-stranded RNA virus belonging to the genus Flavivirus within the family Flaviviridae. Four antigenically distinct serotypes, designated DENV-1, DENV-2, DENV-3, and DENV-4, are capable of causing human disease. The virus is transmitted primarily through the bites of infected Aedes mosquitoes, particularly Aedes aegypti and Aedes albopictus. Following transmission, dengue virus infects dendritic cells, monocytes, macrophages, and other susceptible cells before disseminating throughout the body. Infection with one serotype typically provides long-term immunity against that serotype but only partial and temporary protection against the others. Secondary infection with a different serotype may increase the risk of severe disease through antibody-dependent enhancement (ADE). Dengue virus is one of the most important mosquito-borne viral pathogens worldwide, particularly in tropical and subtropical regions.

Diagnosis
The diagnosis of dengue virus infection is performed through molecular, antigen detection, and serological methods. During the acute phase of illness, reverse transcription polymerase chain reaction (RT-PCR) and detection of the nonstructural protein 1 (NS1) antigen provide sensitive and specific identification of infection. Serological assays, including ELISA-based tests for dengue-specific IgM and IgG antibodies, are commonly used after the acute viremic phase. Interpretation of serological results may be complicated by cross-reactivity with other flaviviruses such as Zika virus, West Nile virus, and Yellow fever virus. Laboratory findings often include leukopenia, thrombocytopenia, elevated liver enzymes, and evidence of increased vascular permeability. Early laboratory confirmation is important for patient monitoring and management.

Symptoms
Dengue virus infection typically develops following an incubation period of approximately four to ten days after the bite of an infected mosquito. Classic dengue fever is characterized by sudden onset of high fever, severe headache, retro-orbital pain, muscle aches, joint pain, fatigue, nausea, vomiting, and a maculopapular rash. The intense muscle and joint pain has led to dengue being nicknamed 'breakbone fever.' Most infections are self-limiting and resolve within one to two weeks. However, a subset of patients develops severe dengue, formerly known as dengue hemorrhagic fever or dengue shock syndrome, characterized by plasma leakage, severe bleeding, thrombocytopenia, organ dysfunction, and circulatory shock. Severe dengue can be life-threatening and requires prompt medical management, particularly in children, elderly individuals, and patients experiencing secondary infection with a different dengue serotype.
