Structure
West Nile virus (WNV) is an enveloped, positive-sense, single-stranded RNA virus belonging to the genus Flavivirus within the family Flaviviridae. The virus is closely related to other medically important flaviviruses, including Japanese encephalitis virus, St. Louis encephalitis virus, Dengue virus, and Zika virus. West Nile virus is maintained in nature through transmission cycles involving mosquitoes, primarily Culex species, and avian reservoir hosts. Humans and horses are considered incidental or dead-end hosts because they typically do not develop sufficient viremia to sustain transmission. Following mosquito-borne inoculation, the virus can disseminate to multiple tissues and, in some cases, invade the central nervous system. Since its introduction into North America in 1999, West Nile virus has become one of the most important mosquito-borne viral pathogens in the Western Hemisphere.

Diagnosis
The diagnosis of West Nile virus infection is performed through serological and molecular methods. Detection of West Nile virus-specific IgM antibodies in serum or cerebrospinal fluid using ELISA is the most commonly employed diagnostic approach and is particularly useful in patients with neurological disease. Confirmation may be obtained through plaque reduction neutralization testing (PRNT) to distinguish West Nile virus infection from other flaviviral infections. Reverse transcription polymerase chain reaction (RT-PCR) can detect viral RNA during the early phase of infection but is generally less sensitive because viremia is often transient and low-level. Cerebrospinal fluid analysis in neuroinvasive disease typically demonstrates lymphocytic pleocytosis and elevated protein concentrations. Laboratory confirmation is important because clinical symptoms overlap with numerous other viral infections and causes of encephalitis.

Symptoms
Approximately 80% of West Nile virus infections are asymptomatic. Symptomatic individuals typically develop illness following an incubation period of approximately two to fourteen days. West Nile fever commonly presents with fever, headache, fatigue, malaise, muscle aches, joint pain, nausea, vomiting, and occasionally a maculopapular rash. Most patients recover completely within days to weeks. A small proportion of infected individuals, particularly elderly or immunocompromised patients, develop neuroinvasive disease characterized by meningitis, encephalitis, or acute flaccid paralysis. Neurological manifestations may include severe headache, neck stiffness, confusion, altered mental status, tremors, seizures, muscle weakness, and paralysis. Severe neuroinvasive disease can result in long-term neurological deficits, respiratory failure, or death.
