Description
Bacterial genus that causes spotted fever and typhus group rickettsial diseases

Structure
Rickettsia is a genus of small, Gram-negative, obligate intracellular bacteria belonging to the family Rickettsiaceae. Members of the genus are highly adapted to life within eukaryotic host cells and rely on host cellular machinery for survival and replication. Rickettsiae primarily infect endothelial cells lining blood vessels, resulting in widespread vascular inflammation and injury. These bacteria are transmitted through arthropod vectors including ticks, fleas, lice, and mites and are maintained in complex zoonotic transmission cycles involving animal reservoirs. Clinically important species include Rickettsia rickettsii, the causative agent of Rocky Mountain spotted fever, Rickettsia prowazekii, which causes epidemic typhus, and Rickettsia typhi, which causes murine typhus. Due to their intracellular lifestyle, Rickettsia species cannot be cultivated using routine bacteriological culture methods and require specialized laboratory techniques.

Diagnosis
The diagnosis of Rickettsia infection is performed through serological, molecular, and immunohistochemical methods. Indirect immunofluorescence assay (IFA) remains the reference standard for serological diagnosis and is commonly used to detect Rickettsia-specific IgM and IgG antibodies. Rocky Mountain spotted fever often presents with a rash that begins on the wrists and ankles and may spread to the trunk and extremities. Some rickettsial diseases produce an eschar at the site of the arthropod bite. Severe infections can result in widespread vasculitis, leading to neurological complications, respiratory failure, renal dysfunction, shock, multi-organ failure, and death if not promptly treated. Early recognition and administration of appropriate antibiotics are critical for favorable outcomes.
