Bacterium that causes tuberculosis

Structure
Mycobacterium tuberculosis is a slow-growing, aerobic, acid-fast bacillus belonging to the Mycobacteriaceae family and is the primary causative agent of tuberculosis (TB). The bacterium possesses a unique cell wall rich in mycolic acids, lipids, and complex glycolipids, which confer resistance to many environmental stresses, disinfectants, and host immune defenses. This lipid-rich cell wall is responsible for the characteristic acid-fast staining properties of the organism. M. tuberculosis is transmitted through airborne droplets generated when individuals with active pulmonary tuberculosis cough, sneeze, speak, or sing. Following inhalation, the bacteria primarily infect the lungs but may disseminate to virtually any organ system. The pathogen is capable of establishing latent infection, during which viable organisms persist within granulomas for years or decades without causing active disease.

Diagnosis
The diagnosis of Mycobacterium tuberculosis infection is performed through microbiological, molecular, immunological, and radiological methods. Microscopic examination of acid-fast stained sputum specimens remains an important diagnostic tool, particularly in resource-limited settings. Culture of M. tuberculosis on specialized media is considered the gold standard for diagnosis and allows antimicrobial susceptibility testing, although growth may require several weeks. Molecular methods such as nucleic acid amplification tests (NAATs) and PCR-based assays provide rapid and highly sensitive detection of M. tuberculosis DNA and can identify drug-resistance mutations. Immunological tests, including the tuberculin skin test (TST) and interferon-gamma release assays (IGRAs), are used to identify latent tuberculosis infection. Chest radiography and computed tomography are often used to assess the extent and location of disease.

Symptoms
Tuberculosis most commonly affects the lungs but may involve lymph nodes, bones, kidneys, the central nervous system, and other organs. Active pulmonary tuberculosis typically develops gradually and is characterized by a persistent cough lasting weeks or months, chest pain, fatigue, fever, night sweats, weight loss, and loss of appetite. Hemoptysis may occur in advanced pulmonary disease. Extrapulmonary tuberculosis presents with symptoms that vary depending on the affected organ system and may include lymphadenopathy, back pain, neurological deficits, abdominal pain, or urinary symptoms. Many infected individuals remain asymptomatic with latent tuberculosis infection, but reactivation may occur years later, particularly in immunocompromised individuals. Without appropriate treatment, active tuberculosis can cause progressive tissue destruction, respiratory failure, disseminated disease, and death.
