Description
Protozoan parasite that causes babesiosis

Structure
Babesia is a genus of obligate intracellular protozoan parasites belonging to the phylum Apicomplexa and the family Babesiidae. These parasites primarily infect erythrocytes (red blood cells) of vertebrate hosts and are transmitted through the bites of infected ixodid ticks. Numerous Babesia species infect mammals, birds, and reptiles, with Babesia microti, Babesia divergens, and Babesia duncani being among the most important causes of human disease. Following transmission, the parasites invade red blood cells and undergo asexual replication, resulting in hemolysis and anemia. Babesia species are widely distributed worldwide and are maintained in natural transmission cycles involving ticks and reservoir hosts such as rodents, cattle, and other mammals. Because of their intraerythrocytic lifestyle, Babesia parasites share certain biological similarities with Plasmodium species, the causative agents of malaria.

Diagnosis
The diagnosis of babesiosis is performed through microscopic, molecular, and serological methods. Examination of Giemsa- or Wright-stained peripheral blood smears remains a classic diagnostic approach and may reveal characteristic intraerythrocytic ring forms or tetrads known as 'Maltese crosses.' Polymerase chain reaction (PCR) assays provide highly sensitive and specific detection of Babesia DNA and are particularly useful in cases with low parasitemia. Serological assays, including indirect fluorescent antibody (IFA) tests and ELISA-based methods, may be used to detect Babesia-specific antibodies and support diagnosis. Laboratory findings often include hemolytic anemia, thrombocytopenia, elevated bilirubin levels, and increased lactate dehydrogenase concentrations. Early diagnosis is important because severe disease can occur in immunocompromised individuals and asplenic patients.

Symptoms
Babesiosis ranges from asymptomatic infection to severe, life-threatening disease. Following an incubation period of approximately one to four weeks after tick exposure, symptomatic individuals commonly develop fever, chills, sweats, fatigue, headache, malaise, muscle aches, and loss of appetite. As the parasites destroy red blood cells, patients may develop hemolytic anemia, jaundice, dark urine, and shortness of breath. Severe disease is more common in elderly individuals, immunocompromised patients, and those without a functional spleen and may result in acute respiratory distress syndrome (ARDS), disseminated intravascular coagulation, renal failure, shock, or death. Coinfection with other tick-borne pathogens, particularly Borrelia burgdorferi, may complicate clinical presentation and disease severity.
