Protozoan parasite that causes leishmaniasis

Structure
Leishmania is a genus of obligate intracellular protozoan parasites belonging to the family Trypanosomatidae and the phylum Euglenozoa. More than twenty Leishmania species are known to cause human disease. The parasites have a complex life cycle involving phlebotomine sand flies as vectors and mammals, including humans, dogs, rodents, and other animals, as vertebrate hosts. Within the sand fly, Leishmania exists as motile promastigotes possessing a flagellum, whereas within mammalian hosts the parasite survives as non-flagellated amastigotes residing primarily within macrophages and other phagocytic cells. Leishmania species are endemic in tropical, subtropical, and Mediterranean regions worldwide and are responsible for a spectrum of diseases collectively known as leishmaniasis, ranging from localized skin lesions to life-threatening systemic infection.

Diagnosis
The diagnosis of leishmaniasis is performed through microscopic, molecular, serological, and culture-based methods. Direct microscopic examination of tissue aspirates, biopsies, bone marrow, lymph node samples, or skin lesions may reveal characteristic intracellular amastigotes within macrophages. Polymerase chain reaction (PCR) assays provide highly sensitive and specific detection of Leishmania DNA and are increasingly used for species identification and confirmation of infection. Serological assays, including ELISA, immunofluorescence, and rapid diagnostic tests based on recombinant antigens such as rK39, are widely used in the diagnosis of visceral leishmaniasis. Culture of parasites in specialized media may be performed in reference laboratories. Accurate species identification is important because disease manifestations, prognosis, and treatment options vary among Leishmania species.

Symptoms
Clinical manifestations of leishmaniasis depend on the infecting species and host immune response. Cutaneous leishmaniasis is the most common form and typically presents with one or more painless skin papules that gradually develop into chronic ulcers with raised borders. Mucocutaneous leishmaniasis may result in progressive destruction of the nasal, oral, and pharyngeal mucosa, causing severe disfigurement. Visceral leishmaniasis, also known as kala-azar, is the most severe form and is characterized by prolonged fever, weight loss, fatigue, anemia, hepatosplenomegaly, pancytopenia, and immune suppression. Without treatment, visceral leishmaniasis is frequently fatal. Post-kala-azar dermal leishmaniasis may develop after treatment of visceral disease and is characterized by persistent skin lesions containing parasites.
