Description
Bacterial genus that causes chlamydial infections in humans and animals

Structure
Chlamydia is a genus of obligate intracellular Gram-negative bacteria belonging to the family Chlamydiaceae. Members of the genus possess a unique biphasic developmental cycle consisting of two morphologically and functionally distinct forms: the infectious elementary body (EB) and the metabolically active reticulate body (RB). Elementary bodies are adapted for extracellular survival and host cell infection, whereas reticulate bodies replicate within membrane-bound intracellular inclusions. The most clinically important species include Chlamydia trachomatis, the causative agent of urogenital infections, trachoma, and lymphogranuloma venereum, Chlamydia pneumoniae, a cause of respiratory tract infections, and Chlamydia psittaci, which causes psittacosis. Because these organisms depend on host cells for energy production and replication, they cannot be propagated on conventional bacteriological media.

Diagnosis
The diagnosis of Chlamydia infection is performed through molecular, serological, and antigen detection methods. Nucleic acid amplification tests (NAATs), including polymerase chain reaction (PCR)-based assays, are considered the gold standard for the detection of Chlamydia trachomatis and provide excellent sensitivity and specificity. Respiratory infections caused by Chlamydia pneumoniae and zoonotic infections caused by Chlamydia psittaci may be diagnosed using PCR, serological testing, or a combination of both methods. Serological assays, including ELISA and microimmunofluorescence tests, are useful for determining exposure history and supporting the diagnosis of chronic or systemic infections. Early and accurate diagnosis is important for preventing complications and limiting transmission.

Symptoms
Clinical manifestations of Chlamydia infections vary depending on the infecting species and site of infection. Chlamydia trachomatis commonly causes sexually transmitted infections that may present with urethritis, cervicitis, dysuria, abnormal genital discharge, pelvic pain, and infertility-related complications; however, many infections are asymptomatic. Ocular infection can result in trachoma, a leading infectious cause of blindness worldwide. Chlamydia pneumoniae typically causes mild respiratory disease characterized by sore throat, cough, fever, bronchitis, or atypical pneumonia. Chlamydia psittaci infection is usually acquired through exposure to infected birds and may present with fever, headache, myalgia, dry cough, and severe pneumonia. Untreated infections can result in chronic inflammation, tissue damage, and significant long-term health consequences.
