Structure
Cytomegalovirus (CMV), also known as Human herpesvirus 5 (HHV-5), is an enveloped, double-stranded DNA virus belonging to the genus Cytomegalovirus within the family Herpesviridae. The virus possesses an icosahedral capsid surrounded by a protein tegument and a lipid envelope containing multiple glycoproteins involved in host cell entry. CMV has one of the largest genomes among human viruses and is capable of infecting a wide variety of cell types including epithelial cells, endothelial cells, fibroblasts, monocytes, and leukocytes. Following primary infection, CMV establishes lifelong latency, primarily within cells of the myeloid lineage, and may periodically reactivate, particularly in immunocompromised individuals. CMV is widely distributed throughout the world, and a large proportion of adults show evidence of prior infection.

Diagnosis
The diagnosis of cytomegalovirus infection is performed through serological, molecular, and virological methods. Detection of CMV-specific IgM and IgG antibodies by ELISA or other immunoassays is commonly used to determine recent or past infection. Molecular methods such as quantitative polymerase chain reaction (PCR) are highly sensitive and are widely used to detect and monitor CMV DNA in blood, urine, saliva, cerebrospinal fluid, and other clinical specimens. Antigenemia assays and viral culture may also be employed in selected clinical settings. In pregnant women and immunocompromised patients, laboratory testing is particularly important for assessing active infection, viral load, and risk of disease progression.

Symptoms
Most cytomegalovirus infections in healthy individuals are asymptomatic or cause mild, self-limiting illness resembling infectious mononucleosis, characterized by fever, fatigue, malaise, sore throat, lymphadenopathy, and muscle aches. Following primary infection, the virus remains latent and may reactivate later in life. In immunocompromised individuals, including transplant recipients, cancer patients, and people with advanced HIV infection, CMV can cause severe disease involving the lungs, liver, gastrointestinal tract, retina, and central nervous system. Congenital CMV infection is a major cause of birth defects and may result in hearing loss, vision impairment, developmental delays, microcephaly, seizures, and other neurological complications. CMV remains one of the most significant opportunistic viral pathogens worldwide.
